- An item with a red asterisk * indicates required information.
- "Meeting" Contact: This is the person to contact if there are questions about this specific meeting. The Meeting Contact person’s name, phone number, and email address are not publicly posted without the person’s permission. Otherwise it is only used by NAWS Email or Phone Servants to help fellows obtain contact information for a specific meeting.
- "NAWS" Contact: This is the person who Nicotine Anonymous World Services ("NAWS") may contact for general World Services communication to keep you up-to-date on Nicotine Anonymous information. (This may or may not be the same person as the meeting contact.)
